On my mission (recent) the mission president shamed about 50% of the missionaries into making belated confessions (I wasn't one of them). I asked several missionaries how the interviews went and they all told me that as part of the interviews, the mission president asked a long list of HIGHLY detailed questions. The list was from church headquarters and the mission president was REQUIRED to ask them. Additionally, in his October 1974 conference address, N. Eldon Tanner's brow beat bishops and stake presidents into conducting "searching" interviews. This address used to be (and might still be) cited in the D&C institute manual in the section about confession. "Searching" (aka inappropriate) interviews are an unwritten rule in the church.
